Medical history and past medications in MIMIV4

Does anyone know where to find medical history and past medications of icu patients in MIMIC 4?

For past medical histories, I found that itemid=225059 and itemid=225811 in mimic_icu.chartevents has 'CAD','CHF','Diabetes - Insulin','Diabetes - Oral Agent','Hypertension',Liver Failure','MI','PVD', 'Renal Failure' . I guess these are past medical histories?

For past medications, I am looking for ACE inhibitors and found itemid=228928 in mimic_icu.chartevents has 'Beta-blockers and/or ACE inhibitors' value. Is this related to the past medication?
