Concept scoring in MIMIC-III echocardiography dataset

Hi,
Just started to work with the MIMIC-III echocardiography dataset and I can't figure something out. Hope someone can help me. I really want to know based on what guideline patients get certain scores on the different concepts. So for instance, someone get's a score of 2 in the category of AV-stenosis, what does it make it a 2 and not a 1 or 3 or -2. Thanks in advance!
