[Improvement] Add support for end modifiers

VexFlow already supports end modifiers, what's missing in mei2vf is the logic to decide if a clef, for instance, has to be rendered at the start of a measure or at the end of the previous measure (or both, e.g. when there's a system break and the clef changes)
