Publish latest version of sqlcommenter-knex to NPM #282

It looks like some of the latest changes in sqlcommenter-knex (like #146) weren't published to NPM.

If I install the package from NPM and look at the code, it still has references to opencensus, while the code on master no longer has those references.

Could you please publish the latest changes to NPM?
