Don't do anything with antora/Makefile yet, as this is still used by the Vulkan-Site repo to build docs.vulkan.org

Will allow validusage.json to contain mappings from VUIDs to the docs.vulkan.org page containing them.
Also adds some missing explicit anchors, and titles on deeply nested anchors.
Pairs with KhronosGroup/Vulkan-Site#93 . Both PRs must be merged to allow building of the site to continue working - merging just one will cause problems.
