Fix Implement Code Action capability #1654
Fix Provide quick-fix for undeclared symbols #1222 (maybe)
- Known issue: diagnostics are not refreshed after creating a new file
Add some more trivial quick fixes
- Add default namespace
- Remove trailing separation
- Add/Remove leading slash
